To strengthen diabetes care for the estimated 75,000 people in Kalulushi District, Zambia.
The proposed project will seek to improve access to diabetes services in Kalulushi District, Zambia (est. population 75,000) through the following components:1. Strengthening of capacity for diabetes care through the provision of standard diagnostic and monitoring equipment, operationalisation of the existing referral system and use of nationally adopted NCD data registry at six health facilities.2. Roll out of a 5-day classroom and on-the-job training package on diabetes management and care targeting healthcare professionals from six health facilities; training to be complemented by subsequent monthly mentoring of trainees by mentors from the District Health Office.3. Improvement of skills for diabetes self-management among diabetes patients and their caregivers/families through implementation of patient education sessions at targeted health facilities by trained HCPs; training to consist of 3-day classroom and 2-day hands-on skills application and to be followed by subsequent formation of patient peer support groups and ongoing support to these.4. Community sensitisation and awareness through the dissemination of health promotion messages on diabetes through various communication channels.5. Advocacy and engagement of district and provincial health authorities through the development, dissemination and discussion of policy briefs focusing on project learnings and challenges about diabetes care.
• 6 clinics strengthened to provide diabetes care and prevention (1 district hospital, 2 health centres, 3 health posts).• 20 doctors, 30 nurses, and 15 clinical officers trained in quality diabetes management. • 50 allied health workers trained in quality diabetes management, referral and diabetes data collection.• 1,000 diabetes patients access quality DM treatment services. • An estimated 3,000 people screened for diabetes.• 500 patients trained in diabetes self-management.• At least 5,000 people receiving health educational messages.
