• To establish two culturally-sensitive satellite clinics responsible for provision of diabetes and foot care in the two target districts• To train local health care providers in diabetes and foot care • To raise awareness of diabetes and foot complications among patients and the general population
The project will be initiated with a preparation phase where the local leaders and community elders will be approached to ensure ownership and promote participation. The preparation phase will also be used to develop extensive training programmes for health care providers and culturally adapted information, education and communication materials. Two training programmes will be rolled out for health care providers working at primary clinics in the target districts. The training programmes will cover both diabetes and foot care and have a strong focus on both theory and practice. After the training the health care providers will be able to provide both diabetes and foot care at the local level. Satellite clinics will be established and equipped with basic diabetes and foot care equipment and equipment for insole production. Electronic patient records will also be introduced. The clinics will hereafter provide improved diabetes and foot care and will receive regular support and supervision from the partner. Outreach camps will be rolled out to provide screening, training and care. Severe cases identified as part of the daily routine at the satellite clinics will be referred to the outreach camps for specialised care. Patient education on diabetes foot self-management skills will be provided. After the camps the follow-up of the patients will be maintained at the satellite clinics. Awareness raising of general population will be done through educational sessions, distribution of materials at local events (traditional ceremonies etc.) and involvement of local community organisations.
